Another task involved examining a fake prescription bottle to extract key details about dosage and usage. Despite the simplicity of these tasks, many patients found it challenging to retain all the critical information, especially after just 10 minutes.
“The way information is presented can significantly affect how well people understand it,” said Stefanie Ferreri, a professor at the University of North Carolina at Chapel Hill Eshelman School of Pharmacy. She noted that even something as simple as taking a diuretic in the morning might be misunderstood. If a patient takes it with dinner instead, they could experience nighttime disruptions, such as waking up frequently to use the restroom. This highlights how nuanced language or unclear labeling practices can lead to avoidable errors.
Health Literacy and the Complexity of Communication
Many participants in the study were experienced healthcare users, often managing multiple chronic conditions. Yet, they still faced difficulties in grasping instructions. “Our study validated the experiences of people who feel lost in the healthcare system,” explained Abigail Vogeley, a research fellow and doctoral student at Northwestern University’s Feinberg School of Medicine. “They’re not alone in their confusion, and the system needs to do more to support them rather than leave them to figure things out on their own.”
Dr. Michael Wolf, professor of medicine and director of Northwestern’s Center for Applied Health Research on Aging, emphasized the challenge of conveying complex information in a short time. “Doctors’ visits are often brief, and the language used can be anything but simple,” he said. “When patients don’t feel comfortable asking questions, critical details can get overlooked.” This lack of clarity, Wolf argued, contributes to errors in medication management, such as incorrect dosing or missed instructions.
Another participant, Dr. Jan Carney, president of the American College of Physicians and professor of medicine at the University of Vermont, pointed out that the way instructions are written plays a key role. “If the language is unclear or the information is not presented in an easy-to-understand format, patients are more likely to misinterpret it,” she said. For example, a label might mention “take with food” but not specify the timing or purpose of the medication, leading to confusion about its optimal use.
The Ripple Effects of Misunderstanding
Errors in medication instructions can have far-reaching effects. Vogeley highlighted that misinterpretations might result in patients taking too little or too much of a drug, combining medications that shouldn’t be mixed, or using them in ways that increase side effects. “Even small details can make a big difference,” she said. “A single misstep in understanding the dosing schedule could lead to a decline in treatment effectiveness or a worsening of symptoms.”
Additionally, the study found that patients often overcomplicate their medication routines. For instance, some might wake up in the middle of the night to take an unnecessary third dose, or spread out their medications so much that their daily activities are disrupted. “How do you sustain taking up to seven doses a day without feeling overwhelmed?” Wolf asked. “That’s a real challenge for many people, and it underscores the need for clearer communication.”
Addressing the Systemic Issue
While the study measured patients’ ability to manage medications and healthcare tasks, it’s not a reflection of individual failure. Instead, Wolf described the findings as a call to action for the healthcare system itself. “The problem lies in how information is delivered, not in patients’ ability to understand it,” he said. “Improving health literacy requires changes in how we structure communication and collaborate with patients.”
Vogeley echoed this sentiment, suggesting that healthcare providers should prioritize simplifying instructions and using consistent labeling practices. “Uniformity in how warnings and directions are written can reduce the likelihood of errors,” she said. “Patients don’t always know where to look for key details, and that can lead to mistakes.”
Combatting Health Misinformation
As health misinformation continues to rise, the importance of clear communication cannot be overstated. “Patients want to do the right thing for their health, but they’re often given information that’s hard to grasp,” Wolf explained. “This study shows that even in routine care, there’s a gap between what’s communicated and what’s understood.”
Carney added that fostering health literacy should be a shared responsibility. “Healthcare providers need to make an effort to explain things in plain language, especially when dealing with medications that have specific timing or interaction requirements,” she said. “Patients also need to feel empowered to ask questions without hesitation.”
The study’s implications extend beyond individual confusion. It reveals a broader issue in the healthcare system: the need to adapt communication strategies to meet the diverse needs of patients. “If we can improve the clarity of instructions, we might reduce the number of preventable mistakes,” Vogeley said. “That’s the real goal here.”
Tips to Enhance Health Literacy
To address this problem, experts recommend a few practical steps. First, patients should ask questions during doctor’s visits to ensure they fully understand their medication. “Don’t hesitate to clarify anything you’re unsure about,” Carney advised. “It’s better to ask now than risk complications later.”
Second, healthcare providers should use consistent terminology across prescriptions and labels. “If a warning is written in one place for one medication and another for the next, patients might miss it,” Wolf noted. Third, simplifying language and avoiding jargon can make instructions more accessible. “Use plain English so that anyone, regardless of their background, can follow it,” Ferreri suggested.
Finally, patients can benefit from reviewing their medication instructions at home. “Going over the details with a family member or writing them down can help reinforce the information,” Vogeley said. By implementing these strategies, the healthcare system can take a significant step toward reducing misunderstandings and improving patient outcomes.
